How does it work?
FORTRAL PLUS Ointment contains the main active ingredient diclofenac diethylammonium, which is a type of medicine known as a non-steroidal anti-inflammatory drug (NSAID). FORTRAL PLUS Ointment works by blocking the action of a substance in the body called cyclo-oxygenase. Cyclo-oxygenase is involved in the production of various chemicals in the body, some of which are known as prostaglandins. Prostaglandins are produced in response to injury or certain diseases and would otherwise go on to cause pain, swelling and inflammation. FORTRAL PLUS Ointment is therefore used to relieve pain and inflammation. All the medicines in this group (NSAIDs) reduce inflammation caused by the body's own immune system, and are effective pain killers. FORTRAL PLUS Ointment applied topically to the skin is used to relieve local pain and inflammation caused by a variety of muscle, joint, tendon and ligament injuries, such as sprains, strains and bruises. It is also used for rheumatism.

FORTRAL PLUS Ointment is a transdermal ointment (gel) constituting Diclofenac salt, Mephenesin, Methyl Nicotinate, Chlorpheniramine Maleate, Turpentine Oil, Menthol and Methyl Salicylate as its main components.
Diclofenac used possesses analgesic properties greater than other NSAIDs in inflammation, Mephenesin is a centrally acting skeletal muscle relaxant used for symptomatic treatment of painful muscle spasm associated with musculoskeletal conditions. Chlorpheniramine Maleate is a antihistaminic with anti allergic effect. Methyl Nicotinate and Methyl Salicylate are Analgesics and Anti Inflammatory drugs used for the relief of pain in musculoskeletal, joint, and soft tissue disorders. Methyl-salicylate is also used locally as a counter irritant. Menthol desensitizes the skin irritation and also acts as a local anesthetic as well as helps in penetration of the other drugs. It dilates the blood vessels of the skin giving a feeling of coldness followed by an analgesic effect. Turpentine oil is used as a carrier of other medicines for faster penetration into the skin.
Being a multi-component preparation FORTRAL PLUS Ointment also has a merit in conditions like endonitis, bursitis and Osteoarthritis, where the pain can be chronic and difficult to treat.

Chlorpheniramine Maleate is a antihistaminic with anti allergic effect useful to counter any allergic reaction to any of the ingredients or also locally in split muscle conditions.
Menthol & Turpentine Oil
On topical application, menthol dilates the blood vessels causing a sensation of coldness followed by an analgesic effect.
Menthol and Turpentine Oil also acts as a penetration enhancer. They increases the penetration of drugs when applied on the skin to give a faster onset of action. Menthol functions as a penetration enhancer either due to its vasodilatation property or since menthol readily penetrates the skin, it may reversibly change barrier-structures in the stratum corneum without affecting blood circulation at the site of application.
Methyl Salicylate & Methyl Nicotinate
Methyl Salicylate/Nicotinate are used topically as counter-irritants. On topical application, they are absorbed through the skin and are applied for the relief of pain in rheumatic conditions and painful muscle or joints. One of the most popular over-the counter group of topical agents are the salicylate-containing anti-inflammatory formulation, recommended for use in wide range of painful or inflammatory conditions in muscles and joints. It has been shown that first-pass metabolism exists in the skin with esterases rapidly hydrolyzing salicylate esters to release the active salicylate in both the epidermis and dermis.
FORTRAL PLUS Ointment is indicated either alone or as an adjunct for the relief from pain, swelling and inflammation due to musculo-skeletal disorders such as sprains, strains, tendonitis, bursitis, hand, neck, should pain, sciatica, muscle stiffness, joint pain, back ache, lumbago. It si specially useful for rheumatoid aches and pains.
FORTRAL PLUS Ointment is contra-indicated in patients with a history of hypersensitivity to diclofenac, aspirin, other NSAIDs, linseed oil, menthol, methylsalicylate or to other ingredients of the preparation.
Adverse reactions possibly involved are mild or moderate local irritation, or a rash. However there is no burning sensation with this ointment.
Mild staining of clothing has been noted.
Dosage and Administration:
Approximately 1" of ointment (gel) should be applied to the affected site 3-4 times daily with rubbing till the film disappears.
